Dear Parents/Carers

RE: NSW Schools Vaccination Program 
NSW Health has partnered with NSW Schools to offer free vaccinations to students in Years 7 and 10.
As part of this partnership, we have upcoming clinic dates to provide this free service at school.

Year 7:  Tuesday 1 April 2025
 
students are eligible for free diphtheria-tetanus-pertussis (dTpa) and human papillomavirus (HPV) vaccines.

Year 10: Monday 31 March 2025
Students are eligible for a free Meningococcal ACWY vaccine.

School Vaccination consent is now online

Online consent is required for their routine school vaccinations.

Vaccination will only be provided at school if consent has been received. 

How to provide consent:

1.       To complete online consent for your child’s school vaccinations, 

visit: https://nswhealth.service-now.com/school

or scan the QR code:

2.       Click the login with ‘Service NSW Account’ button and log in using your 

Service NSW details.
This is the same account you may already use to renew your driver’s licence.
If you don’t have a Service NSW account, refer to Service NSW to create one. 

3.       Update or confirm your personal details in Service NSW as required. 

4.       Complete the School Vaccination Consent Form for your child/ren. You will need to:  

a)      Enter your child’s personal details 

b)      Provide the Medicare card details for you and your child  

c)      Read the linked Parent Information Sheet and privacy statement

d)      Provide consent  

If you or your child do not have a Medicare card, consent can still be provided by requesting a paper-based consent form directly from your child’s school.

Consent can be withdrawn at any time:
Where consent has been given online, please log-in at NSW Health - Engage and follow the prompts to withdraw consent for your child.
Where consent has been given on the physical form, please contact our school prior to the above clinic dates to withdraw your consent.

Physical consent forms can be collected from Willyama High School office.
 
If you do not wish for your child to be vaccinated against a disease, do not collect a physical form and do not provide online consent.

If your child is away and you wish for them to receive a vaccination, there will be a catch-up clinic later in the year, alternatively, you can organise vaccination through your local GP or pharmacist immunisers.
